Panduan Pasien Internasional
The Customer Relations team can assist international patients in extending their Social Visit Pass. The following documents are required for the pass extension:
- Paspor – Pasien & pendamping (asli & fotokopi)
- Akte Nikah / Akte Kelahiran (asli & fotocopy)
- Surat Konfirmasi Pasien dari Petugas Medis atau Konsultan
- Kartu janji temu (asli & fotokopi)
- Kwitansi hotel (asli & fotokopi)
- Saya 55 Formulir
- Letter of Appointment (authorizing the Customer Relations Officer to submit and collect documents on your behalf)
- The new Social Visit Pass will be issued within 2 weeks before the visa’s expiry date.
Patients from selected nationality are provided with “eVISA” services. eVISA is an online application platform that allows foreign nationals to apply for an electronic visa to enter Malaysia from the comfort of their home.
