International Patient Guide
The Customer Relations team can assist international patients in extending their Social Visit Pass. The following documents are required for the pass extension:
- Passport – Patient & companion (original & photocopy)
- Marriage certificate / Birth certificate (original & photocopy)
- Patient Confirmation Letter from Medical officer or Consultant
- Appointment card (original & photocopy)
- Hotel receipt (original & photocopy)
- Imm 55 Form
- Letter of Appointment (authorizing the Customer Relations Officer to submit and collect documents on your behalf)
- The new Social Visit Pass will be issued within 2 weeks before the visa’s expiry date.
Patients from selected nationality are provided with “eVISA” services. eVISA is an online application platform that allows foreign nationals to apply for an electronic visa to enter Malaysia from the comfort of their home.
